Name & Document Changes


The Center Project has information on name and gender changes to official documents

Steps for changing one’s legal name:

  1. Sign and verify a Petition for Change of Name (For Adult Individual) (CAFC401) in front of a notary. Bring the original copy of a valid form of ID (no scans of driver’s license or passport).
    1. Mobile notaries in Boone County
    2. Notarizing at the UPS store 
    3. Notary at Boone County Library
  2. File the verified petition at Boone County Courthouse (or your county’s courthouse).
  3. The judge will issue a certified copy of the approved petition.
  4. Make public notice of the name change once a week for three weeks in a newspaper of general circulation.
    1. File proof of these notices with the Office of the Clerk within ten days of the last notice.
  5. Pay $185 at the time of your appointment. You can also submit an Application for In Forma Pauperis (GN10) to have your fee waived.

Steps for changing sex identifier on one’s drivers license:

  1. Bring the following to a Missouri License Office to change your sex identifier on your drivers license
    1. Fill out the gender designation change request form for driver’s license
    2. If you don’t fill out this form, you can bring one of the following instead:
      1. Medical documentation showing completion of gender reassignment surgery
      2. A U.S. Passport which reflects the gender designation requested by the applicant
      3. An amended U.S. birth certificate which reflects the gender designation requested by the applicant
      4. A court order declaring the gender designation requested by the applicant (may be part of a court ordered name change)
  2. Also bring documents certifying your current name, Social Security number, Missouri residency, and proof of your legal name change
  3. Pay $100 at the time of your appointment. You can also submit an Application for In Forma Pauperis (GN10) to have your fee waived.
